for family members, doctor appointments, and emergency situations.

As a patient with numerous medical conditions, including
Myasthenia Gravis, Mitochondrial Myopathy, Celiac Disease
and many drug allergies, I have learned the importance of
having a Health History Journal on hand to show medical
professionals.  It has helped me correct medical errors and is a
quick reference for my family members, doctors, nurses,
technicians and emergency personnel.  Being an advocate for
our own health care reduces medical mistakes and provides
vital health information when an emergency arises.

with patients in hospitals, nursing homes & residential facilities
much easier, and blank communication picture pages for your
own additional needs.   

A section for note taking, appointment cards and a clear page
holder is located in the very back of the journal for added
convenience.

A
s an advocate for your own health care it is critical to keep
good medical records in order to accomplish open
communications with medical personnel and family members.  
Many medical errors come from patients not knowing what
medications they are on, what dosage the medications are,  
what drug allergies they have, what former surgeries or
medical conditions they have had, what their family medical
history is, or who to contact in an emergency.
Medical errors are mistakes in health care that could have
been prevented. They can occur in hospitals, clinics, surgery
centers, doctors' offices, nursing homes, pharmacies, and
your home. Errors can involve medicines, surgery, diagnosis,
equipment, or lab reports. They are often caused by a lack of
good communication. Medical errors may result in injury or
death. You can help prevent medical errors by being a
well-informed and active partner in your own health care.
The best thing you can do to prevent medical errors is to be
involved in your health care. Learn and know about your
health problem, medicine, and treatment as best you can and
take part in making all decisions about your care.  Show your
Health History & Medical Emergency Journal to everyone
who is involved in your health care. This includes your
doctors, other  health professionals, family,  and  friends.
